Paying a lot for a little

Insurance Company: Blue Cross
I am currently on an employer Anthem Blue Cross plan which has me paying $762.22 monthly to cover my wife and two children. I just received a claim re-cap from Anthem which has me paying the full amount for a Doctors visit.
