you elect continuation of group health coverage on 04/13/09 . it has determined that you are an assistance eligible individual AEI and and eligible for subsidized COBRA continuation coverage will begin on 03/01/09 and can last until 12/01/2009 . Details about the terms and conditions of coverage under ARRA were previously sent to you in your COBRA election Notice.
